| MEMBERSHIP: Termination / Place on Hold - Policy |
Any member requesting termination of their membership, policy states a 30 day written notice is to be submitted. When notice is given you will have 30 days from the last billing date to have access to the facility. Any member requesting their membership to be placed on hold may do so. When placing your membership on hold you will no longer have access to the facility. There are four options when submitting your written request:
1. Stop by THE DIVE SHOP Aquatic Center and complete a Termination / Place on Hold form and execute.
2. Email your request to: tdsacbrkfld@gmail.com
3. Fax your request to: 203-740-9877
4. Mail your request to: THE DIVE SHOP Aquatic Center, 439 Federal Road, Brookfield, CT 06804
There will be no verbal acceptance of either termination or place on hold requests. If the appropriate action is not executed by the member the billing process will continue.
